Please join the Northwest Chapter today by clicking onto the Online Membership Application button (Membership is FREE). You will need to upload a copy of your DD214 with type of discharge with a completed and signed Physician Statement. If you don’t have a DD214 with the type of discharge, press the “Your Free DD214” button and order it online today.
If you are not able to upload the DD214 or Physician Statement today, when in hand, please attach the document(s) and email to matthew.mickunas@gmail; in the Title & Body of your email please write [Attention: Matthew Mickunas, Membership Application]. If local, you may bring them into the Northwest chapter office in Burien, WA (see address at the bottom of the webpage). When bringing in the documents to the chapter office; please enclose in a sealed envelope [Attention: Matthew Mickunas, National Director]. Thank you!
