To become a member of the Paralyzed Veterans of America, Northwest Chapter, just click onto the “Online Membership Application” button below. When filling out the online application, it will ask you to “Select a Chapter”, please select the Northwest Chapter!! After the online application is submitted you may be asked for some additional documents, such as a copy of your DD214 with type of discharge and a signed VA Physician Statement stating your type of spinal cord injury or disease (SCI/D). If you are in need of your DD214, you can order it online by clicking onto the “Your Free DD214” button below.
